MEMBERSHIP CHANGE REQUEST FORM

Use this form for all onboarding, offboarding, or occupancy change requests.
Please provide the Greentown team with 30 days' notice when possible so we can make sure all requests are responded to promptly.

GREENTOWN LABS MEMBERSHIP OPTIONS

Full details and pricing options for Greentown's memberships can be found in our Member Agreement. Below you will find key details about the individual membership options for each team member.

The base membership for any company is one desk membership.

We have three contract options available for each member following your initial base desk membership: desk, fob, or offsite.

Members with onsite contracts (desk or fob) are granted full 24/7 access to the facilities. Offsite members have access to the facilities only during business hours for no more than four days a month and may not work in our prototyping or wet lab.

Restricted areas, such as the wet lab, are only available to members with on-site memberships who have completed the proper training.

Membership Options Overview:

Desk Membership
- 24/7 Access to all of Greentown’s member resources and labs (refer to the table below).
- Member will choose from available desks to set-up their own dedicated workspace.
Fob Membership
- 24/7 Access to all of Greentown’s member resources and labs (refer to the table below).
- No dedicated desk space but these members can use their team's existing desk space.
- This membership is often used for members that will be spending most of their time in the lab.
- A team can only have two fob memberships per one desk.
Offsite Membership
- To keep your remote employees connected to Greentown's many virtual offerings and community an offsite membership is a great option.
- A team can only have two offsite memberships per one desk.
- Offsite members are able to visit Greentown once a week or up to four times a month (beyond that, we ask that they are accounted for with an onsite contract).
- Offsite members are not allowed to work in any of the lab spaces.
- Offsite members must sign-in and pick-up/return loaner fobs at the front desk during the business hours.

Greentown Alumni Membership

Greentown offers an Alumni Membership to members who are transitioning out of Greentown as a way to stay connected to our community and retain access to resources. Alumni Members retain access to office space, community events, GRID, and other member services.

Graduating companies can choose one team member to hold the Alumni membership and retain access, additional team members must be onboarded under alumni, fob, or offsite memberships.

Alumni members have a key fob and can work from any of our common spaces.
Alumni members do not get assigned desks and cannot use our labs.
Alumni members get a mailbox at Greentown.

* Alumni members can get access to analytical equipment of Wet Lab for a fee.

** Ecosystem and Alumni members have access to use conference rooms for up to 5 hours/month. A charge of $15/hour will be applied for additional hours.

*** Limited Software resources do not include access to Solidworks. It does include Altium, Keysight ADS, ANSYS (discounts - not FREE access) and Amazon Web Services Cloud Credits. Mathworks depends on company size and shape prior to MW access.

**** Access to Artisan's Asylum is limited to startup members who are actively renting Prototyping Lab space at 444 Somerville Ave., 28

Dane St., or 24 Park St. Access to Artisan's Asylum is not extended to Corporate Members or Alumni Members.

MEMBERSHIP POLICIES & PROCESSES

Paperwork Required for Greentown Labs Membership

Member Agreement
- Each member company CEO signs the Greentown Labs Member Agreement annually.
- Individual members do not sign the member agreement but must abide but all guidelines on behalf of their company.
Certificate of Insurance pursuant to our COI Guide
Non-disclosure Agreement

These three documents must be signed, up-to-date, and accessible at all times for all active Greentown members.

Onboarding

Please fill the Membership Change form under Access to Forms if you are adding a new employee and Irina Kous will reach out to onboard them on their designated start date.

When we onboard new members, they will be sent:

A welcome email with their GRID and virtual New Member Orientation module links
Once the new member creates their GRID profile, a Wi-Fi pin code will be sent to them to join our secure network
Upon completion of GRID setup and New Member Orientation, their key fob will be left in the company's mailbox within 48 hours
A personalized invitation to join the Greentown Labs Slack

Offboarding

Individual offboarding

Please fill the Membership Change form under Access to Forms if you are removing an employee and Irina Kous will reach out to offboard them on their designated end date.

For individuals being offboarded, please adhere to the minimum required notice periods noted below:

Workspace reductions are subject to the following notice period depending on the level of reduction in subsequent invoices:

<$2,000 - 30 Day Notice
$2,000 - $5,000 - 60 Day Notice
>$5,000 - 90 Day Notice

Company offboarding

Member Companies must provide 30, 60 or 90 days notice before vacating Greentown Labs based on their footprint and membership type based on the value of their last invoice by submitting the Membership Change form under Access to Forms.

Please return all key fobs and parking pass to the dropbox at reception on the member's last day!

Offboarding includes:

Deactivation of key fob
Return of parking pass
Removal from GRID, Slack and all other Greentown Labs owned shared systems
Revoked VPN access

Greentown Labs Notice Terms:

Notice Terms	Minimum Required Notice
Workspace under 1,000 SF	30 days notice
Workspace is 1,000 square feet or greater, but less than 2,500 square feet	60 days notice
Workspace is greater than or equal to 2,500 square feet or more, or Member employs 25 or more persons, or Member has one or more wet lab benches	90 days notice

Prototyping Lab

Requests for prototyping space and all changes in size/shape/scope of work in the Shell Prototyping Lab at 444 Somerville Ave. and the Prototyping Lab at 28 Dane St. must be communicated through the Space Request form under Access to Forms.

Wet Lab

All changes to wet lab bench occupancy must be communicated through the Space Request form under Access to Forms. Greg Ralich will be in touch following your entry to discuss the next steps.

Members must provide a 90-day notice for any reductions in benches.

Members are responsible for providing the Wet Lab team with the most recent copy of their chemical inventory and then following the prescribed decontamination protocol.

Finance

Payments

Card payment: NOTE--Each credit/debit card payment has a transaction fee of 3.5% added to the invoice.

How to pay: Click on Pay by Card, Fill in information, Click on Update payment details

Recurring payments: Each subsequent invoice will automatically be charged to this card as a recurring payment.

If you want to stop card payments, send an email note to Janet Rife from our finance team requesting your card information be removed from GRID.

ACH payment: Must have a pending invoice, Click on Pay by Bank Transfer, Fill in information, Click on Next

Recurring payments: Each subsequent invoice will automatically be pulled from this bank account as a recurring

charge. If you want to stop ACH payments, send an email note to Janet Rife from our finance team requesting

your ACH information be removed from GRID.
